If you are looking to promote healthy nutrition in a neighborhood grade school, you will need to evaluate:

  • Section I- Parts 1 and 2 – The Student population – # of students; # of overweight students; cultural diversity of student population (speak to eating preferences of various cultures); gender distribution.
  • Section I Part 3 – Social Determinants – exercise habits of students (how often is PE offered and what types of exercises are performed, athletics, walking clubs, etc.); socioeconomic levels of students (money for a gym membership or recreation center); does the community have such amenities; transportation to home for afterschool activities; communication of healthy living; educational level of parents; health history of the community in which the school is based; windshield survey- are there walking paths in the community and parks; do the majority of the students have a primary care physician and if yes do they use them; School health programs; are the student smokers; do most use the cafet4eria services or bring lunch; Dietary offerings and support of healthy food choices at the school.
  • Section I Part 4 – Disparities – low socioeconomic students versus affluent students, reside in the outskirts of town versus town center, distribution of ethnicities, etc.
  • Section II & III –Discuss why your indicator is a priority need for your population – Community need: in this example, to address the increasing obesity of the children which can lead to comorbidities such as diabetes, heart disease, low self-esteem, etc.,  and if any of these are already being experienced and to what extent (percentages or ratios). You also need to address the partnerships that could address the issue: WIC, Free lunch program, Support Groups, YMCA or local gyms, youth groups, etc.
